  • Original of the complaint made to the police including the lost passport number. (If the lost passport number is not available, you can obtain it from the Colombo head office or a regional office (Kandy, Matara, Vavuniya and Kurunegala.) of Department of Immigration and Emigration.)
  • If the passport was lost abroad, the temporary travel document (NMRP) used to arrive in Sri Lanka.
  • A fine of Rs. 20,000 in addition to the passport fee (passports lost within one year of issue).
  • A fine of Rs. 15,000 in addition to the passport fee (Passports lost after one year of issue).

This is applicable to all Sri Lankan ordinary passports that valid for all countries.

  • Original National Identity Card with a photocopy that certified by authorised official.
  • Marriage certificate with a photocopy where it is necessary (To confirm the name after marriage)
